Student Forms
| Transfer Credit Form | Get approval for prior transfer credit (business courses only.) |
| ASU Substitution Form | Get approval to substitute an ASU course for one that is listed on your degree plan. |
| Course at Another Institution Form | Get approval to take a course at another institution and it transfer as an ASU course. |
| In-State Residency | Must be completed to apply for in-state residency. |
| Recompute GPA | Must be completed if repeating a course. Complete after course has been repeated. |
| E-Mail Permission | Gives instructor permission to send information to students via e-mail. |
| Recommendation Letter | Gives faculty member permission to write a letter of recommendation for a student. |
| Parental Consent | Must be completed to release student information to student's parents. |
| Readmit Application | Must be completed if returning to ASU after a 7-year or more absence. |
| Transcript Request | May complete to request an official transcript by mail. |
