| Evaluation Form As an intern, it is your responsibility to ensure that your supervisor has a copy of the evaluation form and that it is returned to me when due. Since your supervisor is probably very busy, please be sure to remind him/her at least 10 days or so before the evaluation is due. Check the syllabus to see when evaluations are due.
